Veterinary Experience Verification Form

• Applicants that submit the VMCAS application are required to submit one (1) Veterinary Experience Verification form. This form must be completed by one (1) veterinarian with whom you have accumulated a minimum of 80 hours of veterinary experience.  Please send or fax only one (1) Veterinary Experience Verification form.  We will only review one form so please do not send more than one.   
• Click here  for the Veterinary Experience Verification Form.  The Veterinary Experience Verification Form must be submitted by October 1.
• If you have spent a minimum of 80 hours of veterinary experience with multiple veterinarians within one (1) clinic; we will accept one (1) Veterinary Experience Verification form completed by one of the Veterinarians with whom you worked.
• Send the Veterinary Experience Verification form to:  The Ohio State University, College of Veterinary Medicine, Students Affairs, 1900 Coffey Road, Columbus, Ohio 43210.  You may also fax the form to (614) 292-6989.
• Applicants reapplying from the previous year need not resubmit the Verterinary Experience Verification Form as applications are kept on file for up to one year.
